HOW MUCH WILL THIS COST?
The New York State Prescription Drug Program is a generically enforced program. You will pay your usual copayment for a generic equivalent, or the brand name drug, if a generic is not available. When your doctor has indicated that your pharmacy may not substitute an available generic product, you will pay your copayment PLUS the difference between the cost of the brand and generic drug. If your doctor feels there is a medically necessary reason why you must have the brand name drug, that has a generic equivalent, you may request an appeal to the mandatory substitution requirement. Call 1-(800) 964-1888 to obtain a generic appeal form, If the appeal is approved you will be responsible for only the usual copayment.
